This page of the Uncharted 4 guide has locations of all the notes one of the categories of secrets/collectibles.
You will learn which chapters have notes and how to unlock them.
Move along narrow corridors and stop at the first bigger chamber.
The secret is located on the floor, in the left corner.
Squeeze through a number of narrow corridors, jump down and locate the skeleton presented in the screenshot.
Get there with your jeep and enter a cave hidden behind it.
Exit the car and investigate the cave.
Use your grappling hook on the container presented in the photo.
Pull it down and push it to the right side.
Now you’re free to pull up the upper shelf.
Apart from one of the treasures, you will find the note.
The second notecan be found in the biggest part of the map.
The note is by one of the skeletons (see screenshot).
Head in a straight line towards the left, largest isle of the archipelago.
The note is in the small recess shown in the above screenshot.
You find the second noteon the islet to the left of the largest island.
This is the area with the ship wrecked.
Examine the objects on the beach shown in the above screenshot.
You find the note among them.

Start by jumping over rooftops and find the cottage shown in the above screenshot.
Jump inside across the window and find the note on the table.

Walk into the new building, jump down to a lower level and collect the note from the table.
The first one is near the dam that you had to open with the turning wheel.
In the building, there is the first note.
The other one is near the waterfall that you climb next to.
Climb onto the ledge.
this is where the second note is.
Chapter 18 -New Devon
In this chapter, there are as many as six journal notes.
You find the first one on the bed in the building shown in the screenshot.
The second one is on the skeleton of the hanged woman.
The third one is on the desk.
The fourth one is on the edge of the table by which the pirates were poisoned.
The fifth one is in the neighboring room, in the fireplace.
You receive the sixth one from Elena as a part of the mission.
Chapter 20 - No Escape
In this mission, there is only one note.
The note is next to the bottles.
The game shows that there are two journal notes in this chapter.
However, the last one is available in chapter 21.
It is on the skeleton.
