You will find it inside a hole inside the rock.

Go to the north-east fromReardon Manor.
The chest with anotherdiagramwill be lying inside the cave to its left, where the tiles have been placed.
Theblacksmithcan be found to the south of the city.
Go to the end of the corridor and find the hidden passage with your witcher abilities.
The switch which opens the passage will be on your right.
Now you could get to the chest with anotherdiagram.
